SASSA Form for Care Dependency Grant: Personal Information

Personal Details: Write down your Surname, Full Name, Age, Physical Address, and Postal Code in the spaces provided. 

Child’s Information: Since you’re applying for a Care Dependency Grant for a child, you’ll need to provide their Name, Surname, and Date of Birth in the specified fields. 

Relationship to the Child: Check the box that best describes your relationship to the child. Are you their Parent, Foster Parent, or Primary Caregiver? 

Residence Confirmation: Confirm that the child doesn’t live in a state-funded institution.

Marital Status

Let’s move on to the Marital Status section. Here’s what you need to do:

Choose Your Status: Look at the options provided and check the box that best describes your marital status. Are you Married, Unmarried, In Community, Out of Community, in a Civil Union, or Customary Union? Maybe you’ve never been married, or perhaps you’re Divorced a Widow, or a Widower. If you’ve been Deserted for more than 3 months, there’s an option for that too.

Document Situation: If you’re Married, Divorced, or a Widow/Widower, and you don’t have the necessary documents like your ID, Decree of Divorce, or Death Certificate for your spouse or partner, don’t worry. Just provide reasons for not having these documents. It’s important to explain any missing paperwork so SASSA can understand your situation better.

Sources of Income

Mark X for Income Types: Look at the list of income sources and mark an X in the box next to the ones that apply to you and/or your spouse. Are you getting a Salary or Wage from a job? Do you have Profits or Benefits from a Business or Farm? Maybe you receive Payments from a Trust or Inheritance, or Income from Property Rights.

There are also options like Pension or Annuity, Ex-Gratia Payments, Maintenance Received, Rental Income, and Income from Assets like interest or dividends. Choose all that apply to your situation.

No Income Situation: If you and/or your spouse don’t have any income sources, that’s okay. Just explain how you manage to get by in the space provided. It’s important to let SASSA know if you’re not earning any money so they can understand your financial situation better.

Form Declaration

Now, it’s time for the Declaration part of the form. Here’s what you need to do:

Confirm Information: Declare that all the details you’ve provided in the form are true and accurate to the best of your knowledge. 

Accept Oath: State that you have no objection to taking the prescribed oath and consider it binding on your conscience. 

Form Certification

In the Certification section of the Care Dependency Grant form, you’ll find spaces for important details:

Deponent’s Signature/Thumb Print: Sign your name or provide a thumbprint to confirm the authenticity of the information provided.

Name of Commissioner: Write down the name of the commissioner who is witnessing the form.

Commissioner’s Rank/Force No.: Indicate the rank or force number of the commissioner.

Date and Place: Fill in the date and place where you’re signing the form.

Identity Number: Provide your identity number for verification purposes.
