  • Unmarried: Choose this option if you are not married.
  • In Community/Out of Community: Select one of these options if you are married, indicating whether you are married in the community of property or out of the community of property.
  • Civil Union/Customary Union: Choose this option if you are in a civil union or customary union.
  • Asiatic Religion: Select this option if you are in a marriage governed by an Asiatic religion.

  • Divorced: Select this option if you are divorced.
  • Widow/Widower: Choose this option if you are a widow or widower.

ID Document: If you or your spouse/partner do not have a valid ID document, you’ll need to explain in the “Reason” section.

Decree of Divorce: In case you don’t have the decree of divorce, you can clarify the situation by stating the reasons.

Death Certificate: If the death certificate of your spouse is missing, you’ll need to explain why in the designated “Reason” section.

  • Salary or Wage: If you receive a regular salary or wage from your employment, mark this box.
  • Profits, Withdrawals, or Other Benefits from a Business/Farm (owned): If you own a business or farm and receive profits, withdrawals, or other benefits from it, mark this box.
  • Payments from a Trust or Inheritance: If you receive payments from a trust or inheritance, mark this box.
  • Payment from Property Rights: Mark this box if you receive payments from property rights, such as royalties or leasing income.
  • Pension or Annuity: If you receive a pension or annuity, mark this box.
  • Ex-Gratia Payments Received: Mark this box if you receive ex-gratia payments, which are payments made out of kindness or goodwill rather than obligation.
  • Rental Income: If you earn income from renting out property, mark this box.

Declaration of Assets

In the “Declaration of Assets” section, you’ll need to mark the appropriate box(es) with an “X” to declare your or your spouse/partner’s assets. Here are the options available:

  • Immovable Property Owned/Held under Leasehold (Not Occupied): If you own or hold immovable property under leasehold that is not currently occupied, mark this box.
  • Immovable Property Owned/Held under Leasehold (Occupied): If you own or hold immovable property under leasehold that is currently occupied, mark this box.
  • Investments/Bonds/Loans/Outstanding Debts Due to You: If you have any investments, bonds, loans, or outstanding debts due to you, mark this box.
  • Shares/Share Capital/Interest in Assets in a Company/Institution: If you have shares, share capital, or interests in assets in a company or institution, mark this box.
  • Endowment Policies after Maturity or Cash in Hand: If you have endowment policies after maturity or cash in hand, mark this box.

Declaration

Before proceeding, take a moment to carefully read the declaration statement below. Once you’ve understood it, you’ll need to either sign your name or provide a thumbprint in the designated space. It’s important to note that your signature or thumbprint must be placed in the presence of a Commissioner of Oaths.
